To submit a Mitigation Plan please log into the Portal and choose Mitigation Plans -> Add Mitigation Plan from the left navigation menu.  Detailed instructions can be found by clicking HERE and referring to the SERC Portal Mitigation Plans training document.  The training document also provides instructions on submitting a Mitigation Plan Extension Request and a Mitigation Plan Closure Form.
